Corey Haines 2ea0f0c2c7 fix: remove ./ prefix from plugin skill paths ...

Claude Code plugin validator rejects bare ./ prefixes as escaping
the plugin directory. Strips ./ from all skill paths and source field
so the plugin loads correctly.

Closes #243
