docs(src): update hook composition counts in src and plugin AGENTS.md

Browse Source 
- Core hooks: 39 -> 43 (Session 24, ToolGuard 14, Transform 5)
- Total hooks: 48 -> 52
- Add transform hooks tier to plugin composition table
- Update session/tool-guard hook name lists
