YeonGyu-Kim f8c626086e fix(agent-names): use HTTP-header-safe display names and config keys for API calls (#3138)
Display names with parentheses like 'Atlas (Plan Executor)' cause HTTP
header validation errors in x-opencode-agent-name. This was blocking
Atlas/Prometheus from working via /start-work and auto-retry.

Changes:
- Display names: parens -> dashes ('Atlas - Plan Executor')
- Hooks (start-work, no-hephaestus-non-gpt, no-sisyphus-gpt): use
  config keys ('atlas', 'sisyphus', 'hephaestus') for agent API fields
- auto-retry: use config key instead of display name for promptAsync
- agent-override-protection: handle dash-suffix normalization
- Updated all test expectations to match new format

Closes #3138

YeonGyu-Kim 3e8fd5ff18 fix(start-work): always hand off to atlas regardless of current session agent (#3155)
/start-work is the "execute this plan with atlas" command.
Previously, if the user's current session agent was sisyphus (or any
non-prometheus/non-atlas agent), the hook would keep that agent instead
of switching to atlas. This caused sisyphus to execute the plan.

Fix: unconditionally select atlas (when registered) or sisyphus (fallback).
The current session agent is irrelevant to /start-work's intent.

Adds regression test: running /start-work from a sisyphus session must
always hand off to atlas when atlas is available.

- Add afterAll(() => { mock.restore() }) to 52 test files missing cleanup
- Rewrite create-tool-guard-hooks.test.ts to use spyOn instead of barrel mock
- Fix skill-mcp-manager OAuth tests with missing mockTokens/mockLogin definitions
- Fix start-work hook: show worktree active block on resume with existing worktree_path
- Extract createWorktreeActiveBlock to worktree-block.ts to avoid circular import
- Replace 80-line isolated test runner CI config with single `bun test` command

YeonGyu-Kim 7f846b2da3 fix(start-work): restore atlas handoff
Keep native /start-work resolvable on Sisyphus, but switch the work session back to Atlas when Atlas is registered. Stamp the outgoing agent with Atlas's actual list-display key so config→start-work execution resolves correctly and still falls back to Sisyphus when Atlas is unavailable.

YeonGyu-Kim a3f9eb1337 fix(start-work): fall back to sisyphus without atlas
/start-work should leave plan mode even when Atlas is unavailable. This prevents Prometheus from being persisted into boulder state and keeping resumed work sessions in md-only mode.

YeonGyu-Kim ae05e76ddf fix(start-work): remove worktree setup prompt when unspecified, add strong worktree active instructions
When no worktree is specified in boulder, stop injecting 'Worktree Setup Required'
instructions. When worktree IS present, inject emphatic instructions ensuring the
agent and all subagents operate exclusively within the worktree directory.

- Extract atlas/ into 15 focused modules (hook, event handler, tool policies, types, etc.)
- Split auto-update-checker into checker/ and hook/ subdirectories with single-purpose files
- Decompose session-recovery into separate recovery strategy files per error type
- Extract todo-continuation-enforcer from monolith to directory with dedicated modules
- Split background-task/tools.ts into individual tool creator files
- Extract command-executor, tmux-utils into focused sub-modules
- Split config/schema.ts into domain-specific schema files
- Decompose cli/config-manager.ts into focused modules
- Rollback skill-mcp-manager, model-availability, index.ts splits that broke tests
- Fix all import path depths for moved files (../../ -> ../../../)
- Add explicit type annotations to resolve TS7006 implicit any errors

Typecheck: 0 errors
Tests: 2359 pass, 5 fail (all pre-existing)
