CI test suite exited 1 despite 0 failing tests because process-cleanup.test.ts
assertions left process.exitCode=1 in place. The afterEach hook only reset to
originalExitCode (which starts undefined), not 0, so Bun picked up exitCode=1
on shutdown and reported the shared batch as failing.
Explicitly set process.exitCode = 0 in beforeEach and afterEach so each test
starts and ends with a clean exit state.
Signal handlers covered SIGINT/SIGTERM/SIGBREAK/beforeExit/exit, but a
synchronous throw or a top-level rejected promise terminated the process
without letting TmuxSessionManager (or any other registered manager) run
its shutdown hook. That reliably left orphan tmux panes after an opencode
crash.
Added registration for uncaughtException and unhandledRejection that fan
out through the existing cleanupAll() path, set process.exitCode = 1,
and arm the same 6 second forced-exit guard we use for signals. Test
helpers hold process-level spies so the new tests do not leak listeners
between runs.
Awaiting the tmux callback blocked the prompt for up to 10s (waitForSessionReady downstream). During that window the spawned pane ran 'opencode attach' against an empty session and rendered a blank TUI. Users saw 'pane created but attach not working'.
Start promptWithModelSuggestionRetry immediately after session.create, then invoke the tmux callback as fire-and-forget. The session becomes active before the attach client connects.

agent dispatches background subtasks. The CLI completion checker relies
on `session.children()` + `session.status()` APIs which cannot see
tasks in "pending" state (no session created yet) or tasks whose
sessions are momentarily idle between operations.
This fix bridges BackgroundManager state to the CLI completion checker
using the existing run-continuation-state marker system:
- Add "background-task" continuation marker source
- BackgroundManager writes/clears markers on task lifecycle events
(launch, cancel, complete, crash)
- CLI completion checker blocks exit when marker is active
- Fix todo-continuation-enforcer to also check "pending" task status

The depth limit (default maxDepth=3) was being silently bypassed when
sync-task.ts could not reach the manager's spawn enforcement methods --
the fallback hardcoded childDepth: 1, allowing infinite recursion of
delegate_task calls in degraded environments.

1. The fallback path took the dangerous default silently (no log).
2. There were no end-to-end smoke tests asserting that the depth value
coming back from reserveSubagentSpawn is actually used.

- sync-task.test.ts: add two regression smoke tests:

2. spawnDepth recorded in metadata must equal what reserveSubagentSpawn
returns -- guards against silent fallback to childDepth: 1.
15 new spawn-limits tests + 2 new sync-task tests pass.

OpenCode's fromPlugin wrapper skips Zod validation for plugin tools, so
LLMs can omit required args like description without getting an error.
When Atlas orchestrates and the model omits description, it flows through
as undefined to manager.launch() and background task notifications show
'undefined' for all completed tasks.
Two fixes:
- Add runtime validation for description in delegate-task tool (matches
existing run_in_background and load_skills validation pattern)
- Defensive fallback in notification template: use task ID when
description is missing instead of rendering 'undefined'
The (message.includes("agent") && message.includes("undefined"))
pattern could match unrelated errors like "The agent returned undefined
for the configuration", triggering a false fallback that hides the real
failure.
The two precise patterns are sufficient:
