Filter host config agent aliases with the same protected builtin-name rules used for external agent sources so stale display-name entries cannot replace resolved user-configured models when team mode is enabled.

Atlas was the only agent that bailed early (returning undefined) when
applyModelResolution returned undefined, even if the user had explicitly
configured agents.atlas.model. The general-agents.ts path already handled
this case by falling back to the user's override; sisyphus has its own
explicit-config check. Atlas had neither, so under edge cases (cold
provider cache, no system default, empty availableModels) Atlas was
silently dropped and OpenCode used its built-in default — surfaced as
the hardcoded claude-sonnet-4-6 in the reported bug.
This aligns atlas-agent.ts with the same defensive pattern used in
general-agents.ts: when resolution fails but the user has an explicit
model override, honor it directly instead of dropping the agent.
Adds a regression test covering the scenario where availableModels is
empty AND systemDefaultModel is undefined AND no provider cache exists.
Commit 708891dab fixed most test expectations after the gpt-5.5 model
promotion but missed 13 tests across 6 files that still expected
openai/gpt-5.4 in DEFAULT_CATEGORIES and AGENT_MODEL_REQUIREMENTS.
Updates all remaining stale expectations to openai/gpt-5.5:
- agents/utils.test.ts: atlas/metis resolution, buildAgent category,
override.category expansion (5 tests)
- plugin-handlers/config-handler.test.ts: ultrabrain config resolution

Updates the canonical Anthropic Opus model in every fallback chain
(sisyphus, oracle, prometheus, metis, momus, visual-engineering,
ultrabrain, deep, artistry, unspecified-high), the unspecified-high
category default, the think-mode HIGH_VARIANT_MAP, the Claude Code
alias map, the claude-thinking legacy alias, the context-limit GA
regex, and event.ts fallback strings.
Widens supportsCachedAnthropicLimit to accept both claude-*-4-6 and
claude-*-4-7 so the 1M context cache still applies across the bump.
Regenerates the bundled model-capabilities snapshot from models.dev
and the model-fallback snapshot to match the new source output.
Anthropic API accepts claude-opus-4.6 (dot format) but not
claude-opus-4-6 (dash format). Added anthropic case to
transformModelForProvider to normalize dash-format model IDs before
they reach the provider. Updated model config snapshots and test
expectations to match the new dot-format output.

Hephaestus now uses gpt-5.4 as its default model across all providers
(openai, github-copilot, venice, opencode), matching Sisyphus's GPT 5.4
support. The separate gpt-5.3-codex → github-copilot fallback entry is
removed since gpt-5.4 is available on all required providers.
Keep install-time and runtime model tables in sync, stop OpenAI-only misrouting when OpenCode Go is present, and add valid OpenAI fallbacks for atlas, metis, and sisyphus-junior.

The DEFAULT_CATEGORIES ultrabrain model was updated from openai/gpt-5.3-codex
to openai/gpt-5.4 in a previous commit, but test expectations were not updated.
Updated test expectations in:
- src/plugin-handlers/config-handler.test.ts (lines 560, 620)
- src/agents/utils.test.ts (lines 1119, 1232, 1234, 1301, 1303, 1316, 1318)
Sisyphus can now fall back through Kimi and OpenAI models when Claude
is unavailable, enabling OpenAI-only users to use Sisyphus directly
instead of being redirected to Hephaestus.

Align runtime defaults, tests, docs, and generated artifacts with the newer GPT-5.4 baseline. Keep think-mode and prompt-routing expectations consistent after the model version bump.
The test 'hephaestus is created when github-copilot provider is connected'
had incorrect expectation. github-copilot does not provide gpt-5.3-codex,
so hephaestus should NOT be created when only github-copilot is connected.
This test was causing CI flakiness due to incorrect assertion and
missing readConnectedProvidersCache mock (state pollution between tests).
Also adds cacheSpy mock for proper isolation.
Revert test name and assertion to original behavior per PR review feedback.
The test now correctly expects Atlas to respect uiSelectedModel instead of using its own fallback chain.

Hephaestus now appears when any of its providers (openai, github-copilot, opencode) is
connected, rather than requiring the exact gpt-5.2-codex model. This allows users with
newer codex models (e.g., gpt-5.3-codex) to use Hephaestus without manual config overrides.
- Add requiresProvider field to ModelRequirement type
- Add isAnyProviderConnected() helper in model-availability
- Update hephaestus config from requiresModel to requiresProvider
- Update cli model-fallback to handle requiresProvider checks
Add claude-opus-4-6 as the first anthropic provider entry before
claude-opus-4-5 across all agent and category fallback chains.
Also add high variant mapping for think-mode switcher.
